What is the difference between health plan (plan de saude) and health insurance (seguro medico) ?

I am reading a lot of articles about health insurance in Brazil and sometimes I see the term "health plan", and sometimes "health insurance". Are they the same ?

I read that Health plans were restricted locally and restricted to a doctor network, whereas health insurance is national. Is that true ?

Is health plans more dependant on the state and health insurance more private ?

Can you have health plan and health insurance or should it be health plan OR health insurance ?

What about international private health insurance ? If a foreigner has got his international health insurance, can he avoid having a health plan ?
